Label

  1. Review/Update User Information (top left of screen)
  2. Select the Provider Type from the dropdown list
  3. The Primary Contact/Authorized Signatory can assign other users if necessary:
    1. If the provider received a Notice of Limitation, a preparer can be assigned to complete the Request for Revision report
    2. Click on the Assign Users link at the left of screen to do so.
  4. Click on the Open Enrollment Forms button
